In Kentucky, Oral Surgeons are enrolled as Dentist along with adding the specialty of the Oral Surgeon. However, there are certain oral surgery codes that are reimbursed under a Physician number and that’s when they will require to enroll as PT 64 Physician as well.
Additionally, some of them are dually licensed as a Dentist and a Physician and if they are, then they need to enroll under each discipline. Kentucky does specify direct and indirect owners, or controlling interest of 5% or more in our ownership disclosure question. They have to provide all the names, addresses, SSNs and FEINs for all individuals with that criteria. Additionally, we include the definitions of Indirect Ownership Interest, Ownership Interest, and Person with an ownership or control interest so there is no confusion about what we are asking them to provide.
