Beneficiaries aging out of the EPSDT Private Duty Nursing Services
“Beneficiaries who are receiving services though the EPSDT Private Duty Nursing (PDN) Program, age out at the age of 21. These beneficiaries are considered medically fragile and require a higher level of home care that is medically necessary than on any other Medicaid Program within our state, such as Home Health.
How are other States handling transitioning the medically fragile that age out of this program and the family wants to maintain them in the home setting?
What can be offered to the patient and family after age 21 to assist with the medical care that they continue to need regardless of their age?
